This is a sample form that submits an applicant via the API.


Feel free to copy the source it is a clean html form.
The dropdowns are populated and the form submission is handled by javascript
The javascript is dependant on Jquery and can be downloaded here /Scripts/FullerAPI.js

This form does not work in MS Edge Browser.
